Course Description
Pharmaceutical care is the responsible provision of drug therapy to achieve definitive results that improve a patient's quality of life. The content of the course includes the processes of a pharmacist's collaboration with patients and other healthcare professionals in designing, implementing and monitoring a therapeutic plan that will produce specific therapeutic outcomes for the patient. The course includes the systematic implementation of recording and interpreting patient information, documenting the treatment plan and desired treatment goals for the patient, evaluating the treatment plan, designing a continuous follow-up plan, distribution of medications, providing correct medication recommendations to the patient, and identifying and eliminating possible medication errors.
